Discharge Arrangements

• Whether a patient is fit for discharge is a clinical decision of the healthcare team
• Ward staff will verify with the case doctor for the detailed discharge arrangement of each patient
• Ward staff will verify the parents’ ID before they can bring the child home; an authorization letter is needed if the parents are to designate someone else to pick up their child
• Relevant documents will be provided upon discharge, such as discharge slip for fees payment and medications collection, discharge summary, follow-up appointment slip, and sick leave certificate
