Medical Social Services Unit
Introduction
We strive to offer quality and continuous care for our patients and their families.
Our medical social workers would provide them with timely psycho-social intervention to cope with or solve their problems arising from illness, trauma or disability.
As a member of the clinical team, medical social workers would link up medical and social services to facilitate the patients' recovery and rehabilitation in the community.
Scope of Services
- Psycho-social assessment
- Counselling to patients and their families
- Pre‐admission and discharge planning and follow‐up services
- Offer financial or material assistance e.g. waiving of medical charges, application for trust funds, referral for social security, and purchase of medical appliances, etc.
- Collaborate with the medical team to formulate treatment and rehabilitation plan
- Make referral for rehabilitation services and community resources for patients and their families
- Advocate patient's rights and benefits
